Certification Request
Request
a Copy or Original Replacement Certification or Card
If you previously completed a course through MediTrain, LLC, and you
need a replacement card or certification, please read the following:
- If you need an original
replacement card, certificate, or ODJFS In-Service Training Form,
a $5.00 processing fee is required in advance.
- If you are a
Childcare Provider and need an original replacement of both
the card and the ODJFS In-Service Training Form, you must
complete two separate requests and pay two separate processing fees.
- If
you would like a photocopy mailed, faxed, or e-mailed to you, there
is no charge.
Please
have the following information ready and click Get Started to
place your replacement certification order request:
- The
name of the course you completed.
- The
month and year you took the course (if known).
- Your
credit card (if you are ordering an original and wish to pay
online).
